Farmington Memorial Park can assist you with the preparation of the VA Form 40-1330 "Claim for Standard Government Headstone or Marker" to obtain a military marker. We are able to receive the shipment and place it on their gravesite. If the family wants it installed in concrete, there will be an additional charge - which will be quoted at the time of need (as the work will be done by a third party).
